    The Steps are Easy...

1. After your three night hospital stay, you can transfer to our Rehabilitation Center at Wesley Village.

2. You will then be evaluated by your doctor and our highly trained Therapy Department to develop a program to meet your needs. (Therapy Options)

3. Now you are ready to begin your Therapy!

4. Your program is evaluated weekly with new goals set for you.

5. Your Therapy continues with a target goal set to transition back to your home.

6. A home visit with you and your Azer Therapist is scheduled to eliminate any problems that you might encounter when returning home.

7. A discharge date is scheduled.

8. Wesley Village staff will make a complimentary home visit.
